Steps for sending in the forms:

1. Fill out each form
2. Save the forms to your computer (can be done by pressing the print button and saving the form as a PDF)
3. Click the button below or email forms to appointments@buffaloprairiedental.com
4. Attach the forms to the email
5. Include your first and last name and clinic location in the email header
